Darvon: dextropropoxyphene.   In the UK, dextropropoxyphene and co-proxamol are now discouraged from general use; and, since 2004, preparations containing only dextropropoxyphene have been discontinued.  Darvon is still available in the U.S. 

Darvocet-n (n for acetaminophen).  30mg / 600mg to 100mg / 650mg, respectively   

Both propoxyphene and its metabolite norpropoxyphene, have local anesthetic effects at concentrations about 10 times those necessary for opioid effects [resemble lidocane].  In this respect, norpropoxyphene is more potent than propoxyphene, and they are both more potent than lidocaine. 

Low rate of side effects, the mole wt of davocet is 35% greater than darvon because it is a sulphate vs an HCl.  Peak plasma levels at 2-2.5 hours & is absorbed slower than the HCl.  Half-life of 6-12 hours, whereas norphopoxyphene is 30-36 hours.  Norpropoxyphene has substantially less central-nervous-system-depressant effect than propoxyphene but a greater local anesthetic effect, which is similar to that of amitriptyline and antiarrhythmic agents, such as lidocaine and quinidine.

Propoxyphene is a mild narcotic analgesic structurally related to methadone.   

Opioid effect of lowering blood pressure through vasodilatation (should not be administered to patients with circulatory shock).  The respiratory depressant effect includes carbon dioxide retention and secondary elevation of cerebrospinal fluid pressure, which may be markedly exaggerated in the presence of head injury.

Angiotensin Converting Enzyme (ACE) Inhibitors:  The hyponatremic and hypotensive effects of ACE inhibitors may be diminished by the concomitant administration of aspirin due to its indirect effect on the renin-angiotensin conversion pathway.

Dr. Cleveland cited a multi-institutional study [of cancer patients] that showed pain at a level of three interferes with a patient’s enjoyment of life. At level four, it interferes with work. At five and six, it affects activity, mood, and sleep. At seven, it hinders a patient’s ability to walk. At eight, pain makes it difficult for a patient to relate to loved ones. A study of 527 patients at M. D. Anderson showed that one in five outpatients rated their pain as seven or higher. A lot of these symptoms that occur together seem to be related to an inflammatory response to the therapy for cancer,” Dr. Cleeland said. The insult of therapy generates cytokines, which appear to cause not only pain but also lethargy, loss of interest, diminished learning capacity, and other problems in animal models. If these results translate to clinical studies, as expected, the treatment of symptoms could be simplified. “Patients with seven or eight symptoms may not need to take seven or eight drugs,” Dr. Bruera said.

The efficacy of paracetamol when used in a combination form with weak opioids (such as codeine) has been questioned by recent data studies; the small amount of data available have made reaching a conclusion difficult. Combination drugs of paracetamol and strong opioids like morphine have been shown to reduce the amount of opioid used and improve analgesic effect as well as discouraging overuse of addictive opioids due to APAP's potentially toxic effects.^[20]

 Paracetamol use for fever in the first year of life was associated with an increase in the incidence of asthmatic symptoms at 6–7 years, and that paracetamol use, both in the first year of life and in children aged 6–7 years, was associated with an increased incidence of rhinoconjunctivitisand eczema.^[32]  Paracetamol hepatotoxicity is, by far, the most common cause of acute liver failure in both the United States and the United Kingdom.^[10][42]Paracetamol overdose results in more calls to poison control centers in the US than overdose of any other pharmacological substance.^[43]
